How to Apply
The application period opens on December 1, 2011 and closes on February 27, 2012 at 11:59 p.m. PST. Applications can be secured at your local high school. Alternatively, applications can be downloaded from this page.
Application Process
All applicant information is confidential. An application checklist (below) should be downloaded and utilized to make sure all aspects of your application have been properly addressed. Complete applications according to the instructions below.
Application Submission
Once completed, submit the application to your high school school counselor's office by the date and time specified above. Alternatively, you can submit the application by mail to the address listed on the application, or submit it electronically to scholarship@pvjuniors.org

- You must have Adobe Acrobat Reader in order to fill out this application.
- Fill the application by tabbing from field to field
- You may save and continue the application on your own computer.
- Finally, print out the application and secure the requested verification signatures and submit as instructed above.

Directions for Microsoft Word Download
- You must use Microsoft version 97 or above.
- You must save the black application form in Word to your own folder, disk/CD or hard drive. Otherwise, your application will not be saved.
- After saving the blank application to your personal file, fill in the form.
- Print out the application and submit it as per the instructions above.
- If you prefer, print the blank application and complete it by hand using a black or blue ink pen or typewriter.
